2012 USQA National Quoit Tour Sign-up

There are four Scheduled Tournaments for the 2012 Tour Season. 

You will need to complete a separate form, each mailed in a separate envelope,  for each tournament you are planning to attend.  

Please print out the desired sign-up form, fill out the form completely, and remit payment along with the completed form.
